Our Home Care Services aim to assist and promote the overall well-being of Individuals in the comfort and familiarity of their home environment.
We understand the uniqueness of each Individual and provide in-home care based on the individual's needs. Person-centred care plans are designed with the input of the Individuals, their families and relevant health and social care experts. The goal is to support the Individuals in achieving optimal health outcomes, maintaining independence, and enhancing their quality of life and preventing unnecessary hospital admissions. Our team of dedicated and experienced caregivers is always available to support you or your loved ones, attending to their personal health and social care needs, while collaborating with other health and social care experts/professionals to guarantee the continuity of the best possible comprehensive care.
Personal Care
Our personal care services are designed to assist and support Individuals in activities related to their activities of daily living. The services are person-centred in their approach to ensure individual receive the necessary support while respecting their dignity, privacy, preferences, autonomy and enhance quality of life. Our dedicated staff of Carers will provide personal care services, which may include assistance with washing, shower, bathing, dressing, grooming, hair care, oral care, skin care, toileting and incontinence care.
Medication Management/Adminstration
As part of our care services, we assist with medication management, including storage, organisation and administration of medications as prescribed, monitoring adherence, giving medication reminders and coordinating with health and social care providers for medication refills. We collaborate closely with health and social care professionals, such as doctors, nurses, or pharmacists, to ensure proper medication management. Our Carers will communicate any changes in the Individual condition, report medication side effects or concerns, and seek guidance or clarification as needed.
Meals
Meal preparation plays a crucial role in ensuring that Individuals receive adequate nutrition and maintain their overall well-being. At Open Doors Healthcare Services Ltd, we understand nutritional requirements vary depending on the Individual's condition, their individual needs and preferences. This allows us to work closely with the Individuals' dieticians and families to tailor the best approach to meet the unique requirements of each Individual and promote their overall health and well-being through proper nutrition and hydration. Our staff are more than ready to assist with meal preparations, individual health promotions/advice on nutrition, nutritional monitoring, while ensuring the highest level of food hygiene and safety are maintained.
Mobilisation/ Transfer Assistance
During care visits, our team of carers may assist with mobilisation and transfer for Individuals who may have difficulty moving or transferring independently. As part of our home care services, the staff support the needs of the Individual based on the user care plan that outlines the specific assistance required for mobilization and transfers as recommended by occupational therapists. The care plan should consider the individual's capabilities, any necessary mobility aids or equipment, and any precautions that need to be taken. Our staff are trained on moving and handling to ensure the safety and comfort of the Individual.
